Since his birth name was TITUS and your poem was written by TIMOTHY Smart, I reckon TIMOTHY TITUS is the best option.
You can call him 'Tim' when you like him; 'Timothy' when you don't; and 'Titus' to impress the neighbours, posh friends and (since they are also the names two books from the New Testament) all members of the clergy!
However, as most self-respecting cats decline to come when their called, it really doesn't matter!!
